Username		Your SL name here
Module or chassis		The name of the module or the name of the mech chassis in brackets
Severity(1-5)		Put a number here based on the best match below. 1 for cosmetic issues E.G. Missing textures, sounds, animations etc. Anything that is an issue that does not prevent the module from working. 2 for no update warning 3 for minor or infrequent issues with the module 4 for severe issue with the module 5 for the module being completely non functional or cannot be updated.
